Trilivy Review: Products, Pay Plan and the Sales Decline

October 28, 2026 by Nate Bianco Leave a Comment

Trilivy Review: Is it worth joining?

Trilivy is the new name for OPTAVIA, the weight-management network marketing business owned by Medifast. In this Trilivy review, I’ll cover its products, compensation plan, costs, and income disclosure, along with the question that immediately caught my attention: how did a business with almost $1.6 billion in annual sales fall below $400 million just three years later?

The company is still operating, but it’s working through a major turnaround. Whether you’re considering the products or the business, understanding that history matters more than getting excited about a new name.

Research updated October 28, 2026. This review concerns the U.S. business. Prices and policies can change.

What Is Trilivy, and Is It the Same as OPTAVIA?

Yes. OPTAVIA became Trilivy in July 2026. Its parent company is still Medifast, which trades on the New York Stock Exchange under the symbol MED. Trilivy isn’t an independent startup or a brand-new business opportunity.

Independent distributors are called coaches. They help customers follow the company’s programs and can earn commissions on qualifying product sales. Coaches can also build teams and qualify for additional compensation, so yes, Trilivy uses a network marketing model.

Who Founded Trilivy?

The roots go back much further than the Trilivy name. Medifast’s early history is associated with physician William Vitale and meal-replacement products distributed through doctors. Accounts commonly date those roots to 1980, while Medifast’s current company timeline lists 1981.

The direct-selling business developed as Take Shape for Life around 2002–2003. Bradley MacDonald and Dr. Wayne Scott Andersen are identified in historical company materials as co-founders of that operation. It became OPTAVIA in 2017, then Trilivy in 2026.

Medifast’s current president and CEO is Nicholas Johnson. So when you’re evaluating Trilivy, you’re looking at an established company changing its approach, rather than founders launching something from scratch.

What Does Trilivy Sell?

Trilivy combines packaged nutrition products, structured eating plans, independent coaching, community support, and a healthy-habits framework. Its Reset, Refine, and Renew positioning addresses different stages of the customer’s health journey.

Its Fuelings include bars, shakes, soups, oatmeal, and other portion-controlled foods. For example, the Reset 5 & 1 Plan involves five Reset Fuelings and one Lean & Green meal each day. That meal requires additional food beyond the packaged kit.

At the time of research, the Reset 5 & 1 kit listed at $480 for 140 servings. The site also displayed a $384 introductory Premier+ autoship price, subject to its terms. That isn’t a promise that every future order costs $384.

From a business perspective, I’d look beyond the food alone. Customers can find shakes and bars elsewhere. The question is whether the complete program gives them enough value to choose it and continue buying. That deserves more attention than a compensation-plan headline.

You can believe in the products and still wonder, “Where am I going to find people who actually want this?” Your friends and family are only so big a list.

How Did Sales Fall From $1.6 Billion to Under $400 Million?

This wasn’t a small slowdown. Medifast’s reported annual revenue went from approximately $1.599 billion in 2022 to $385.8 million in 2025, a decline of about 76% over three years.

Here is the recent history, rounded:

  • 2020: $934.8 million.
  • 2021: $1.526 billion.
  • 2022: $1.599 billion.
  • 2023: $1.072 billion.
  • 2024: $602.5 million.
  • 2025: $385.8 million.

These are Medifast’s reported revenues, not estimates from an MLM ranking site. Its OPTAVIA business accounted for all company revenue in 2023, 2024, and 2025. The earlier figures are also parent-company totals, rather than sales under the new Trilivy name.

Customer Acquisition Became Harder Before the Rebrand

Medifast’s second-quarter 2024 SEC filing provides a useful explanation. Management said the downward trend in active earning coaches began in the third quarter of 2022, driven by declining customer numbers. It attributed those pressures to changing social media algorithms and inflation affecting consumer spending.

That matters because it means the decline didn’t suddenly start with Trilivy. The underlying business was already having difficulty bringing in customers while it was still called OPTAVIA.

GLP-1 Medications Changed the Competitive Market

In the same filing, Medifast said the decline became sharper beginning in the second quarter of 2023 as GLP-1 weight-loss medications became more prevalent. The company described those medications as a competitive threat that changed how consumers thought about weight loss.

Its 2025 reporting continued to identify difficulty acquiring clients and the adoption of GLP-1 medications as challenges. In other words, the company wasn’t just competing with another shake or meal plan. It was responding to a major change in the category.

The Active Coach Network Also Contracted

Medifast reported 60,900 active earning coaches in the fourth quarter of 2022, compared with 16,100 in the fourth quarter of 2025. Those are quarterly active earning counts, not the total number of people ever enrolled.

With a coach-led model, customer acquisition and the size of the productive coach network are closely connected. The filings document both declining sales and a shrinking active network. They don’t give us enough information to assign an exact percentage of lost sales to each cause.

So the answer isn’t simply “the products stopped working” or “everyone quit.” The documented explanation is a combination of customer-acquisition difficulties, spending pressure, changing marketing conditions, and new competition, alongside a much smaller active coach network.

Is Trilivy Growing Again?

Not on a year-over-year revenue basis yet. In its October 26, 2026 results, Medifast reported third-quarter revenue of $72.0 million, down from $89.4 million a year earlier. Revenue for the first nine months was $224.5 million, compared with $310.7 million in the same period of 2025.

There is some balance to that picture. The company reported $172.5 million in cash and investments and no debt, and management pointed to improved coach productivity. The rebrand, new products, compensation changes, and training platform are part of its turnaround effort.

A turnaround effort isn’t the same as a completed turnaround. I’d watch whether customer demand and sales begin growing again, rather than treating a new name as proof that the problems are solved.

How Does the Trilivy Compensation Plan Work?

The official U.S. plan effective August 1, 2026 combines customer commissions, team bonuses, and leadership bonuses. Compensation Volume, or CV, can be lower than retail price; Qualifying Volume determines eligibility.

Customer Commissions and Repeat Orders

The base customer commission is 10%. With at least five ordering entities, monthly frontline qualifying volume unlocks additional bonuses:

  • 1,500: 10% extra, totaling 20%.
  • 3,000: 15% extra, totaling 25%.
  • 4,500: 20% extra, totaling 30%.
  • 6,000: 25% extra, totaling 35%.

These apply to eligible compensation volume, not necessarily the customer’s full payment. Repeat orders can produce recurring commissions when qualifications are maintained.

Team and Generational Residuals

Senior Coaches can qualify for a 3% team-building bonus; Executive Directors and above can qualify for 4% on eligible team volume.

Regional Director and higher ranks unlock leadership generations, up to six: 4%, 4%, 3%, 3%, 2%, and 2%. Generations are defined by qualified Executive Director organizations, not ordinary sponsorship levels. Those percentages apply to different volume blocks, not a combined 18% on every order.

The plan also includes customer-acquisition and rank-advancement bonuses. Coaches receive no compensation on their own purchases.

My View on the Residual Income Potential

I like the leverage network marketing can provide. Once you’ve developed people who build their own businesses, the organization can grow beyond your personal efforts. That’s a big part of why the model appealed to me when I started online.

But a residual percentage only becomes meaningful when there are real customers ordering products. And repeat purchases aren’t guaranteed. I’d ask how customers transition after reaching a weight goal, what they keep buying, and why they would remain customers over time.

My bigger point is this: don’t spend weeks comparing little differences between pay plans while putting almost no time into learning how to reach interested people. A fair plan matters. So does having someone to show it to.

How Much Does It Cost to Become a Trilivy Coach?

The official Coach Business Kit was listed at $199 when I researched this review. The company describes resources including a personalized website for 12 months, digital guides, client-support tools, business education, a smart scale, and access to mentorship and community.

Confirm the current annual renewal charge and certification requirements before enrolling; I couldn’t verify the latest renewal amount from a current official page. Also budget for any optional products, events, travel, advertising, or outside tools you choose. A starter kit price isn’t your complete business budget.

If you become a product customer too, review the autoship terms, ongoing price, shipping, and cancellation process separately from your coach enrollment.

How Much Do Trilivy Coaches Actually Earn?

The company’s 2025 U.S. income disclosure covers all independent U.S. coaches, including those who earned nothing. It reports annual gross earnings before expenses:

  • 23.28% had no earnings.
  • 69.79% earned $1,000 or less.
  • 82.21% earned $2,500 or less.
  • Approximately 1.37% earned more than $50,000.

The middle coach fell within the $250.01 to $500 annual range. That’s a range, not a published exact median. These results reflect 2025, before the new Trilivy plan, and don’t predict what you will earn.

In my experience, many people enroll in network marketing and do little or nothing to build. That isn’t a measurement of effort at Trilivy; it’s something I’ve seen across the industry. You can offer support and a starting point, but you can’t make someone take action.

That’s why I focus on two things I can control: continuing to meet new prospects and giving the people who join a useful way to get started. I don’t build a plan around every person becoming a leader.

Enrolling a new coach is exciting. But when they ask, “How do I find people?” where will you send them? You shouldn’t have to create a marketing course from scratch.

What Training and Support Does Trilivy Offer?

Medifast says it launched a new coach-developed Coach Hub training platform alongside the Trilivy changes. The company also promotes coaching resources, community, and mentorship. Ask your prospective sponsor to show you the actual training and how new coaches use it.

I’d specifically ask about finding customers. Learning the products and supporting a client are important skills. You also need a way to keep meeting people who may want those products.

One experience sticks with me here. A woman on one of my teams wanted to learn the marketing method I was using because she saw my results. I taught her what I knew, but she struggled with it for months. Later she learned organic Facebook marketing, found something that fit her, and eventually started out-recruiting me.

That’s why I prefer training that gives people more than one way to generate leads. What works for me isn’t necessarily what you’ll enjoy or stick with. And when you get stuck, being able to ask an experienced marketer a question can be far more useful than watching another video alone.

Outside training doesn’t replace Trilivy’s product education or policies. Before using an outside funnel or sharing marketing resources with a team, check the rules governing advertising, claims, and cross-promotion.

Who Are the Top Marketers Associated With Trilivy?

Business For Home lists Dan and Megan Valentine and David and Terri Miller as top earners associated with Trilivy/OPTAVIA. That’s a third-party listing, not a verified current company ranking.

I couldn’t independently confirm their current earnings or who presently has the largest organization. I’d avoid treating estimated top-earner checks as a forecast for a new coach, especially while the business and compensation plan are changing.

What Concerns Should You Consider?

The sales decline is the most obvious business concern. I’d also look carefully at product affordability, repeat-customer demand, the income disclosure, and the work required to qualify for leadership compensation.

There has also been litigation concerning OPTAVIA’s subscription practices. A California court record from June 2025 states that Zeller v. Optavia had settled and records dismissal of the action without prejudice. That is separate from the company’s sales decline; I haven’t found evidence establishing it as the cause. Read current subscription terms rather than assuming a discounted first order tells the whole story.

On the product side, make sure the program fits your needs and discuss suitability with your healthcare professional. Independent coaching isn’t a substitute for medical care.

Trilivy Review: Potential Strengths and Drawbacks

Potential Strengths

  • An established parent company with publicly available financial reporting.
  • A structured product-and-coaching program rather than products alone.
  • Customer and team commissions that can recur with eligible sales.
  • A published income disclosure that includes coaches with no earnings.

Potential Drawbacks

  • A major multiyear revenue decline and a turnaround still underway.
  • A substantial customer food budget, depending on the plan and discounts.
  • Multiple volume definitions and qualifications to understand.
  • Low annual gross earnings for most coaches in the published disclosure.
  • The need to keep acquiring customers in a highly competitive market.

Is Trilivy Worth Considering?

Trilivy may be worth researching if you value structured nutrition, coaching, and community, and you’re prepared to learn both client support and customer acquisition. It is less suited to someone expecting easy team duplication or income simply because the company was once doing over a billion dollars in sales.

My first question would still be: would you want the products if you never earned a commission? Then I’d ask whether other customers would see enough value to keep buying.

I wouldn’t let the sales decline go unexplored, but I also wouldn’t claim it proves the company is about to disappear. The facts show a much smaller business trying to adapt. Its future results will tell us more than the rebrand itself.

Whatever you choose, learn skills you can keep. Lead generation and building your own permission-based email list have been central to my business since the early years. Companies can change, and so can marketing methods. Being able to reach new people, follow up, and help willing reps learn gives you something useful beyond one product line.

You can’t decide who on your team will take action. But you can learn how to keep reaching new prospects and give willing reps somewhere to learn those same skills.
